پرسشنامه حاضر در جهت تکمیل پایان نامه کارشناسی ارشد با عنوان ؛ « رابطه بین کیفیت خدمات و وفاداری با توجه به نقش میانجی رضایتمندی ارباب رجوع در اداره اوقاف و امور خیریه شهرستان رشت » ؛ در دو بخش طراحی شده است. بخش اول مربوط به سؤالات جمعیت شناختی، بخش دوم مربوط به سؤالات کیفیت خدمات، وفاداری و رضایت مشتریان می باشد. از آنجاکه دستیابی به نتایج صحیح و علمی منوط به اظهارنظرهای ارزشمند شما است، لذا خواهشمندم با پاسخگویی دقیق به پرسشهای مطرح شده و انتخاب گزینه های مناسب، ما را در انجام مطلوب تر این پژوهش یاری فرمایید. لذا فرم های پرسشنامه بدون نام بوده و به صورت محرمانه، توسط محقق، صرفاً برای انجام تحقیقات استفاده خواهد شد. قبلاً از بذل توجه و صرف وقت ارزشمند تان سپاسگزاری می نمایم.
